We have contacted other medical practices to ask them to provide ongoing care for our patients. The practices are: Maine Coast Memorial Hospital, Blue Hill Memorial, Mount Desert Island Medical Center and Trenton Health Center. Your records will be available to these practices or any provider of your choice. Please feel free to stop by the clinic anytime to fill out a records release form so we can forward your records on as soon as possible. It has been a privilege to provide medical care for each of you over the many years.
